Your First Visit

We look forward to meeting you and your family during your very first visit to our office! For your convenience, we allow patients to fill out and submit their new patient paperwork virtually before their first appointment. If you would rather fill out your forms in our lobby, please plan to arrive an extra ten minutes earlier. If you have dental insurance, please bring your insurance card and a valid ID with you to your first visit. Should you ever need to cancel an appointment with us, we ask that you notify our team by phone at least 48 hours in advance.
